Primary Purpose
As an Occupational Therapist, you’ll assess, recommend, and provide therapeutic activities, compensatory strategies, home care services, education, environmental modifications, and adaptive aids. Your goal will be to develop, improve, recover, or maintain the basic ADL (Activities of Daily Living) and IADL (Instrumental Activities of Daily Living) roles needed for our participants to continue living independently at home and within their community.
Key Roles
- Conduct comprehensive assessments and care planning in participants’ homes.
- Provide treatment to enhance or maintain ADL and IADL functions.
- Educate participants, caregivers, and facilities (AFC/SNF).
- Supervise Certified Occupational Therapy Assistants (COTA).
Duties and Responsibilities
- Perform initial and periodic assessments in participants’ homes and develop participant-centered care plans.
- Conduct Level of Care Determination assessments per Michigan state guidelines.
- Perform functional cognitive assessments to tailor participant care plans.
- Assess the need for adaptive/durable medical equipment and make recommendations.
- Regularly evaluate the necessity for occupational therapy and home care services.
- Develop and implement therapy plans including I/ADL training, therapeutic exercises, DME education, and caregiver training.
- Conduct fall risk assessments and create intervention plans.
- Participate actively in care planning and quality improvement activities.
- Document care and maintain participant records.
- Provide training and supervision to COTAs and other staff.
Qualifications
- Doctorate or Master’s degree in Occupational Therapy from an ACOTE®-accredited program.
- Valid Michigan Occupational Therapist license.
- Minimum one year of experience with the elderly or long-term care population.
- Strong communication, problem-solving, and organizational skills.
- Proficiency with MS Office Suite.

- CPR certification (or willingness to obtain).

- Current immunizations or medical documentation for waiver.
- Reliable transportation, valid Michigan driver’s license, and vehicle insurance.
Work Environment
- Visible presence in the center, subject to frequent interruptions.
- Communicate effectively with staff, participants, caregivers, and external organizations.
- Flexible working hours, including weekends and holidays when necessary.
- Potential exposure to TB, AIDS, Hepatitis B, and unpredictable home environments.
Physical Demands
- Handle the mental and emotional stress of the role.
- Ability to work independently with flexibility and integrity.
- Capable of pushing, pulling, moving, sitting, or standing for long periods.
- Lift up to 50 pounds as needed.
