Physical Therapist (PT)

Job Summary: The physical therapist will play a pivotal role in maximizing a patient’s progress toward achieving functional goals through evaluation/assessment, treatment planning/ implementation, education, and communication with minimal supervision. The physical therapist will help restore function, reduce pain and prevent disability following disease or injury in accordance with the principles and practices of physical therapy and within Harvest Health and Rehab policies and procedures. The physical therapist will also establish rapport with patients/families, staff and physicians to maintain a positive working relationship.
Duties/Responsibilities but not limited to :
- Provide direct patient care, review patient’s medical history, evaluate functional needs and establish goals for effective outcomes
- Diagnose patients by observing their movements and listening to their concerns
- Design and implement an individualized plan of care for patients based on a thorough assessment of potential for improvement, re-asses as needed and establish a home exercise program for maintenance.
- Outline clear goals for patients and the expected outcomes of the plan of care
- Use a multitude of treatment options including but not limited to exercises, stretching, equipment, dry needling, modalities, hands-on manual therapy and more to manage patient’s pain, increase mobility, and prevent further pain and injury
- Document evaluations, treatment goals, and plan of care in accordance to specific insurance standards
- Educate patient and family members
- Work collaboratively with professionals from other disciplines and communicate effectively
- Conduct therapy services within scope of practice
- Practice company and departmental policies and procedures, objectives, quality improvement programs, safety, environmental, and infection control standards
- Provide direction and leadership to staff and act as a resource to promote physical therapy in the community
- Enhance professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops while keeping up to date on professional literature
- Maintain an active physical therapy license through the Board of Physical Therapy Examiners

- Knowledge and skills to care for the physical and developmental needs of patients across the lifespan
- Ability to remain calm in emergency or crisis situations
- Compassion, empathy and positive attitude
Qualifications/Education/Experience:
- Graduate of an accredited school of physical therapy
- Active state of Indiana license from the state Board of Physical Therapy Examiners or ability to obtain one.
Physical Requirements:
- Duties require extensive standing and walking
- Requires lifting, positioning, pushing, and/or transferring patients
- Duties require frequent reaching, stooping, bending, kneeling, and crouching
