The Couleecap Housing and Community Services department is hiring one Medicaid Enrollment and Housing Navigation Specialist to provide intake, enrollment and assessments to individuals and families. Responsibilities include needs assessment, service plan development, monitoring, support, advocacy and referral.
Duties & Responsibilities
- Supports and upholds Couleecap’s mission to fight poverty and promote self-sufficiency for individuals by helping to identify needs, mobilize resources, and provide quality services.
- Recruit and enroll program participants. Determine eligibility via interview and verifications. Process referral documenations.
- Provides assessment/individual service planning and ongoing follow‑up for clients.
- Develops individual strategic plans with clients to achieve goals, establishing short‑ and long‑term goals.
- Provides housing navigation supports to clients, including searches for housing, completing housing applications, facilitating contact with potential landlords, and helping clients to understand their lease
- requirements.
- Provide information of community resources that would benefit households in their needs
- Acts as advocate for clients receiving case management services; making referrals to other services as appropriate, which may include arranging transportation, scheduling appointments, encouraging follow up care.
- Maintain up to date information on referral sources and caseload capacities.
- Communicate with case managers and other services providers to establish ongoing supports
- Attends meetings and trainings.
- Keeps records and prepares reports.
- Works as a member of a team to provide effective, quality services.
- Assist in evaluation of programs. Assist in development of programs and services.
- Ability to understand, appreciate, and interact with people from all cultures or belief systems.
